Twins have always fascinated astrologers. Two children born to the same mother, in the same place, often only minutes apart - and yet they grow into two different people. Astrology looks at twins from two angles: what in a chart points towards a twin birth, and why twins with nearly the same chart live different lives.
This guide covers both, calmly and without overpromising. Astrology shows tendencies; only a doctor and a scan can confirm a twin pregnancy.
The twelve signs are grouped by their nature:

Dual signs carry the idea of "two". Gemini is even symbolised by a pair of twins, and Pisces by two fish. Almost every classical rule for twin births leans on these four signs.
The classical verses on twins are preserved in Tamil and Sanskrit texts. In plain English, they say:
From Varahamihira's Brihat Jataka: when the Sun is in a four-footed (quadruped) sign - the verse names Aries, Taurus, Leo, Sagittarius and Capricorn - and the other planets are strong in dual signs, with a dual-sign Lagna, the child may be one of twins.
A verse attributed to Jambu Maharishi says: when the lord of the 3rd house (the house of siblings) and the lord of the Moon sign are together in one sign, joined or aspected by Rahu or Ketu, a twin birth is indicated. The same verse adds that if the planets involved are male, the twins are boys; if female, girls; and if mixed, one of each.
The same source adds a second rule: when the Lagna lord, the 4th lord and the Moon occupy one house at birth, the person may be a twin.
For couples, the classical rule looks at the 5th house (children). When the 5th lord is in a dual sign, or the 5th house is strongly linked with dual signs in the husband's or wife's chart, the couple may have twins.
These combinations are old and interesting, but they are indications, not proof. Many people have dual signs prominent in their charts and are not twins, and twins are born without every rule being met. Use them as one thread of a full chart reading.
This is the question people ask most. If the chart is the same, why are the lives different? The answer is that the chart is not quite the same.
On average a sign rises in about two hours, so the Lagna (ascendant) moves roughly 1 degree every 4 minutes. Twins born 10 minutes apart usually share the same Lagna sign, but the degree differs by two to three degrees. Near the edge of a sign, the second twin can even have a different Lagna. Each divisional chart cuts a sign into smaller parts, so it changes faster:

So two twins can have the same Rasi chart but a different navamsa Lagna, a different D10 Dasamsa for career, and almost certainly a different D60. Astrologers who work with twins rely heavily on these finer charts.
The Moon moves about half a degree an hour, so in 10 minutes it moves around 5 to 6 minutes of arc. A nakshatra is 800 minutes of arc long, so the balance of the first dasa changes by about 1/150th of that dasa's length. For a 20-year Venus dasa that is roughly seven weeks; for a 6-year Sun dasa, about two weeks. Every later dasa and bhukti shifts by the same amount, so the twins' periods begin on slightly different dates all through life.
Classical astrology also accepts that effort, choices and circumstances shape how a chart expresses itself. Two people with similar charts can still make very different use of the same opportunities.
Classical astrology links twin births with the dual signs - Gemini, Virgo, Sagittarius and Pisces - and with specific combinations of the Sun, the 3rd lord, the Moon-sign lord, the Lagna lord, the 4th lord and the nodes. These are tendencies, not certainties. Twins' charts look alike but are never identical: the Lagna degree, the divisional charts and the dasa dates all differ, which is why astrologers use exact birth times and the finer charts to read each twin as an individual.
Classical texts list combinations that point towards twin births, mainly involving dual signs (Gemini, Virgo, Sagittarius, Pisces). They show a tendency, not a certainty. Only medical scans can confirm a twin pregnancy.
The main birth chart is usually almost the same, but not identical. Even a few minutes change the Lagna degree, and often the navamsa and other divisional charts, as well as the exact dasa balance.
Their charts are not exactly the same. Divisional charts such as the D9, D10 and D60 change within minutes, and astrologers use these finer charts, together with the exact Lagna degree, to explain the differences.
The dual signs - Gemini, Virgo, Sagittarius and Pisces. Gemini, the sign of the twins, is the best known, but all four are counted in the classical rules.
